What is Crestor?

This medication belongs to a class of drugs called HMG-CoA reductase inhibitors, or “statins.” It is used with a proper diet to help lower “bad” cholesterol and fats (such as LDL and triglycerides) and raise “good” cholesterol (HDL) in the blood. It may also be used to lower the risk of stroke, heart attack, and other heart problems in people with diabetes and coronary heart disease. It works by reducing the amount of cholesterol the liver produces.

AstraZeneca manufactures the brand in Australia, Canada, India, and Turkey. The generic version is available from Canada and the United Kingdom.

Available Strengths and Sizes

The brand and generic versions are both available in 5mg, 10mg, 20mg, and 40mg tablet strengths.

Contraindications

Do NOT take this medication if any of the following conditions exist:
• Active Liver Disease: Ongoing liver problems or unexplained high levels of liver enzymes found during blood tests.
• Hypersensitivity: A known allergy or hypersensitivity to rosuvastatin or any of the inactive ingredients in the tablet formulation.
• Pregnancy and Lactation: A confirmed pregnancy or current breastfeeding, as cholesterol is essential for fetal development, and the medication may pass into breast milk.

Precautions & Warnings

The following conditions warrant a detailed discussion with a healthcare provider before starting this medication, as they may need to be adjusted or avoided to ensure patient safety:
• Alcohol Use: Excessive alcohol use or a history of liver disease.
• Asian Ancestry: Specific ethnic backgrounds, particularly those of Asian descent, have been shown in studies to have significantly higher systemic exposure to rosuvastatin, which may require a lower starting dose.
• Advanced Age: Patients aged 65 and older may be at higher risk for statin-induced muscle injury (myopathy).
• Hypothyroidism: Uncontrolled underactive thyroid, a known risk factor for developing muscle-related side effects during statin therapy.
• Renal Impairment: Severe kidney disease; dose adjustments are necessary to prevent drug accumulation in the system.
• Skeletal Muscle Effects: A personal or family history of hereditary muscular disorders or a history of muscle toxicity from other cholesterol-lowering medications.

Note: Notify your doctor if you experience changes in blood sugar levels or cognitive effects such as confusion or memory loss.

Drug Interactions

Understanding how Crestor interacts with other substances is crucial for safe and effective treatment. The following medications may need close monitoring or dosage adjustments when taken together.
• Antacids: Taking antacids that contain aluminum and magnesium hydroxide at the same time as Crestor can reduce the concentration of rosuvastatin in your blood. To avoid this, take antacids at least 2 hours after your Crestor dose.
• Cyclosporine: This immunosuppressant significantly increases rosuvastatin levels in the body (up to sevenfold). Due to the high risk of muscle toxicity, the use of Crestor is often restricted, or the dose is kept very low when taken together.
• Gemfibrozil and Other Fibrates: Combining it with gemfibrozil should generally be avoided. Using it with other fibrates (such as fenofibrate) increases the risk of severe muscle breakdown (rhabdomyolysis).
• Protease Inhibitors: Certain medications used to treat HIV or Hepatitis C—such as ritonavir, lopinavir, atazanavir, or simeprevir—can cause rosuvastatin to accumulate in the body. Your healthcare provider may need to adjust your dose if you are taking these antiviral therapies.
• Warfarin: It can increase the blood-thinning effects of warfarin (Coumadin), raising the risk of bleeding. Patients on this combination should have their INR (International Normalized Ratio) monitored closely, especially when starting or adjusting the dose of Crestor.

Other Notable Interactions:
• Colchicine: Using this gout medication with Crestor may increase the risk of muscle pain and weakness (myopathy), especially in older patients or those with kidney problems.
• Niacin: Taking high doses of niacin (1 gram per day or more) with statins can increase the risk of skeletal muscle effects.
• Oral Contraceptives and Hormone Replacement Therapy (HRT): It can increase blood levels of estrogen and progestogen when taken with birth control pills, which may increase the risk of hormone-related side effects.

This medication should be stored at room temperature away from heat, light, and moisture.
